IMMIGRATION EXAMS

PROCEDURE

* Client will be informed that the fee will include office visit, nurse examination, civil surgeon review, lab draw fee and completed I-693 forms IF all conditions are met. The fee does NOT include labs or vaccinations. Referrals to other providers may incur additional fees.

*Complete exam according to instructions.

*Inform patient labs will be sent out, and they will be notified once everything has been reviewed and processed (typically 5-7 business days).

*Prepare certified copy by ensuring civil surgeon signed off on documentation and all results are present. Enclose official copy in envelope, stamped with USCIS ONLY and seal twice with tape or seal, initialing over each. Provide photocopy of all documents to patient in separate envelope.

*Patient MUST pick up copies. Notify patient to not tamper with or open envelope.

NOTES

*Clients must be informed they can get immunizations anywhere, but that any titer or vaccination result MUST be notarized prior to submission to the civil surgeon

*Translators must be provided by client, and must sign I-693 form

*Clients MUST pick up official copy and personal copy of I-693 (cannot mail). Instruct patient they MUST NOT open the USCIS copy

*Duplicate copies are signed by civil surgeon and are available for $85

*Labs are to be dated on date of PROCESSING, not date of draw

*Complete all blank spaces with N/A or NONE
